Assign both to diagnosis code and the E code for:

Fracture of right ulna due to fall from motorcycle. Patient was passenger on the back of the motorcycle and was brought in by ambulance immediately following the accident.

The answer is S52.201A, V28.1zzA

Rationale: The Alphbetic Index main term is Fracture, traumatic, subterm ulna, resulting in code S52.201 with a seventh character of "A". The Alphabetic Index of External Causes main term is Accident, subterms transport, motorcyclist, passenger, noncollision accident, nontraffic, resulting in V28.1 with a seventh character of "A".
