EKG with incident to visit

Can a mid level provider sign and bill for EKG (93000) service or a physician has to sign that report? And if the physician sign it can the NPP bill for it with her "incident to" visit on the same day?

There are a few questions in this with their own answers:
A mid level can sign and bill for the EKG IF it is in their scope of practice (state specific)
Whoever does the interpretation should bill for the EKG.
EKG does not apply as "incident to", meaning the Mid level does not need a cosignature, unless their state does not allow it in scope, in which case the physician is responsible to provide their own I/R of the EKG.
