Recurrent problems

In regards to Box A in Medical Decision Making, the number of problems to examiner, we are trying to define when a recurrent condition is considered a new problem vs an established problem.

For example- recurrent UTIs if a pt has a UTI that continues to return is this an established problem, or a new problem to the examiner? When is that decision made to determine that this is now a new problem? Could it be after the initial course of treatment and the pt is symptom free, and then returns again a month later for the same UTI symptoms? Or is this an established problem because it would be the same problem & same symptoms?
