Provider has created a template for HIV visit: What elements can be validated for the HPI?
I get duration for how long pt has had dz, modifying factors for the meds being taken can I use no symptoms as an associated sign and symptom if the documentation indicates patient having no symptoms. What else can I count to get to the extended?

HIV
Patient has been having no symptoms
Possible medication side effects include none. No side effects from current meds.
The patient has disclosed HIV status to parent(s), other
Overall quality of life is unable to work, but able to live independently
Patient presents with asymptomatic HIV infection
Most recent CD4 count was unknown
Most recent viral load was unknown
HIV was diagnosed in 2013
HIV risk factors include unprotected sex
Regarding sexual activity, patient is sexually active
and has sex with men only, has one partner She had tubal ligation and history of tubal ectopic..
CD4 nadir was unknown
Current meds include Genvoya (elvitegravir/cobi/TAF/emtricitabine)
Number of missed doses in the past month: 3
Barriers to adherence include developmental delay
